At our company hackathon I led a team focused on this issue. What we landed on is in the future looking at code diffs is going to continue to go down. AI can write the code and AI can review the code and we are going to see bug rates go down at a "tactics" level.
What matters are the high level decisions in the code such as "Deleting an account removes the row for good", "Email becomes the user’s permanent ID", etc.
We imagined a completely new concept for code reviews where you review the decisions not the code diffs. Also if you want to review the code it is grouped by concept not by random file diffs.
This is coming from someone that likes manually coding btw but our PR size and velocity is increasing too fast to meaningfully review all the diffs forever.
